Unlocking them is sort of like unlocking cosmetic skins. You right-click them in your inventory and add them to the novelty wardrobe which has its own tab in the hero panel. Novelties are broken up into types (Tonics/Stuff You Can Hold/Toys/Chairs/Instruments), so in the novelties tab you can put whichever novelty you want in the slot that matches its type. Then, out in the world, there's a small button at the bottom right of your skill bar you can click to select one of your five "active" novelties. Whichever one of them is selected at the time is the one that activates when you tap your novelty button/keybind, and you can change what's active whenever you feel like it. Mounts work the same way.
