I've encountered this twice within the past month or so, before that not a single time in ages. But it could as well be a coincidence. The last time I ended up in a rock in Tangled Depths (!) while fighting the Chak Gerent and using Guardian greatsword skill 3. I had to use the waypoint to free myself.

Have you checked for overheating issues?

You mean putting the game into Windowed Mode, then dragging it to your other screen and maximizing it there to (Windowed) Fullscreen Mode doesn't work?

(Nevermind, I figured out the cause for the glitch I mentioned in this post. It was the remain of a shader I was no longer using - it somehow still appeared in the .ini, oops!)

If this happens again and you are using a HDD: try defragmentation. Also, a disk check couldn't hurt.
